Discovery Call Deposit Policy

To reserve your Discovery Call with Dr. Tess Fab, a $150 deposit is required at the time of booking.

This deposit will be applied toward your first appointment if you and Dr. Tess Fab decide to move forward with care.
If, during the Discovery Call, it’s determined that our practice is not the right fit for your needs, your $150 deposit will be refunded in full to your original payment method.
If you cancel or do not attend your scheduled Discovery Call without providing at least 24 hours’ notice, the deposit will be non-refundable.

By scheduling your Discovery Call, you acknowledge and agree to these terms.

Before we move forward, please take a moment to review and confirm the following statements. This helps ensure that our practice is the right fit for your needs and that we can provide you with safe, appropriate, and supportive care. Thank you for taking the time to look these over.

If you find that you do not meet these criteria at this time, please know that you are always welcome to reconnect in the future if your circumstances change.

In the meantime, we encourage you to continue receiving support and care through your primary care physician or other licensed healthcare providers in your area.

In order to continue to the form and taken to the booking site, you must meet all four requirements and check all boxes.

I understand that Dr. Tess Fab Integrative Medicine is a self-pay practice. My credit card will be charged on the day of my appointment. I may request a Superbill to submit to my insurance for potential reimbursement; however, Dr. Tess Fab is not responsible for any denied or unpaid insurance claims.

I confirm that I will be physically located in the state of California at the time of my telehealth appointment, or that I have a specific exception to discuss directly with Dr. Tess Fab.

I have an established primary care provider and understand that Dr. Tess Fab Integrative Medicine consultations are complementary to, and not a replacement for, the care I receive from my primary care physician.

I understand that Dr. Tess Fab currently provides care for children, teens, and adults under age 65. Because Dr. Tess Fab does not participate in Medicare, I confirm that I am under 65 years old and not enrolled in Medicare.
